CDC Animal Care Facility Reservations

This reservation request form is only for foreign vaccinated dogs from high-risk rabies countries traveling to JFK Airport in New York. Our team will reach out approximately 2 weeks prior to the requested arrival date. Submitting a form does guarantee a reservation. A reservation is not complete without written reservation confirmation from our facility.

NOTE: We require at least 45 days notice for all Animal Care Facility (ACF) reservation requests. A draft Certification of Foreign Rabies Vaccination and Microchip form may be used to submit your request since it is only valid for 30 days. We are not currently accepting any expedited reservation requests. If your arrival date is not available on the form below, we are not able to accept any reservations for that date.*

UPDATE: Dogs arriving regulations should expect to stay at least one night before being cleared for entry.*
