CDC Animal Care Facility Reservations

This reservation request form is only for foreign vaccinated dogs from high-risk rabies countries traveling to JFK Airport in New York. Submitting a form does guarantee a reservation. A reservation is not complete without written reservation confirmation from our facility.

*NOTE: We require at least 21 days notice for all Animal Care Facility (ACF) reservation requests.

*Effective January 12 the CDC removed several serology labs. You can check here to make sure sure your serology lab is approved by the CDC: Approved Rabies Serology Laboratories for Testing Dogs

In-Cabin: Your dog flies with you inside the plane — usually under the seat in a carrier.
Excess Baggage: Your dog flies on the same plane as you or under a flight ticket, but in the cargo hold of the plane (a temperature-controlled area for animals).

Manifest Cargo: Your dog flies as official cargo, airline treats your dog's travel as a freight shipment.
